Specifically, this study determined the profile of the respondents in terms of age, educational attainment and number of deliveries. The self care activities of the respondents were identified. Moreover, the existence of the significant relationship between profile of the respondents and their self- care activities were also investigated. Furthermore, the action plan aims to generate knowledge about self- care specifically to the post partum mothers. This study utilized a descriptive- correlational research design. There were 125 respondents that were post partum mothers that have undergone a normal spontaneous vaginal delivery. Purposive sampling technique was used. A researcher- made questionnaire was utilized using simple percentage, weighted mean and Chi- square test of relationship. The findings revealed that a majority of the respondents belonged to the ages 18-21 years old. Their educational attainment in majority was in high school level and almost all are the first timers in terms of delivery. The action plan would provide support in the attainment of the respondent's self- care deficits, especially in improving their interest in advance specialization studies.
